Do a web search on wheelchair ramp slope and you will find a lot of sources. We did a charity project a few years ago and I was in charge of the wheelchair ramp - about the same as your situation, about 3' rise with a right angle turn in the middle. There are different guidelines, but most call for 1" rise over 1 foot, so that means a 35' ramp. 5' of width is more than enough, but you need to leave plenty of room for the turn. We used a roll of roofing paper to give a good, none-slip surface.
