Have you checked the water pump for wear... you might try roughing up the flat belt pulley on the water pump a bit with emery cloth.

Also... there were a couple of belt choices just a little different in length for that application. Have you looked at the tightener to see if it's centered between the marks for travel on the tightener? If the belt is a little too long that could be causing the problem.
