Here's another sinario when a clutch will disinigrate. If you are in a very low range and push the clutch in and let it roll fast downhill the gear reduction becomes gear multiplication on the clutch. The clutch can see 15000 rpm and explode. This happens a lot on 4X4 trucks when I guy eases over the top of a hill and then pushes in the clutch to let the vehicle run out at the bottom. Be the same effect with the low gears on a tractor so watch that clutch on the down hills.
