I have a situation I just finished dealing with. I am finding that I"m dealing with these same types of situations more and more. Different areas but inner related. This last situation dealt with a person recomending the deadline of a fleet (6) of forklifts due to a manufctures weld. Apparently the under bracket thats welded to the fork (to keep fork on the carrage) was bent way back almost ripped off. After I received this info from our safety rep I initiated my own investigation, which didnt last long. Come to find out from the guy"s in the lift shop (maint) this happens several times a year. The warehouseman wrap a chain around the fork and pull material out of the vans. Well guess were all the force is placed? Ding Ding Ding! You got it. Now I"m not the smartest guy around nor the dumbest but thats something that should have been discovered at the onset, especially if your interviewing the driver who did it!I take some photos and send then to our safety rep and fill him in with the story. I also inform him of the rather large pin locaten in the back counter weight, and include the fact if a chain is being used then they best slide it in a steel pipe or they may be eatting links for lunch. Just a story that I deal with all to often!Take Care.
