This is truly a tragic thing. The prayers of me and my family will be for you and yours at this time.
You are right - accidents happen when the tasks we perform become just "routine". We let our guard down for just a moment - and then it happens. My best description is that it's just like an ambush - by the time you realize what has happened - it's all over.
