Sorry to hear about your incident. Isn't it funny how a perfectly good machine can injure itself so fast? Had a fan blade fly off on a 105 JD combine once. The machine started vibrating so I stopped and searched the whole thing over without finding anything. Then I noticed the vibration when the engine was reved with the separator disengaged. I shut it down and finally found the culprit. I have no idea why the blade didn't hit the rad and there were no marks anywhere to tell me where it exited the machine. On the way to the salvege yard to get a different fan, I saw a neighbor's combine in flames. I didn't feel so bad about my little breakdown after that.
