Was the old oil cooler faulty? I think porobably not?
Assuming it was not, you have an oil leak,probably from the head area, leaking into the cooling system. The bubbles from the head gasket indicate a leak from the combustion chamber.
Has the head been refitted and a) not tightened down in the right sequence, b) not tightened down sufficiently, or c) not retorqued, if necessary.?
So yes it is serious, or could be and it may be connected to the first problem, but possibly only as a second symptom of a single fault - a leaking head gasket.
It could, of course, indicate a warped head, cracks, or low liner height (if the engine is sleeved).
If allowed to continue, sooner or later you may get coolant in the oil, a completely blown gasket, gases into the coolant, etc.
It needs addressing now before further damage is done, not left and hope it goes away.
Btw you should have flushed the coolant system with water, then a suitable flushing agent, then with water again before refilling with the correct coolant.
