I agree with the hand placement, and only pulling upward, however I don't believe I am sold on the standing to the side. If it kicked back it could come around and catch your leg. I have also had crankes fly off (on kick backs, when cranking Farmalls) and if you are standing off to the side, the crank could hit you. I crank standing in front (insuring my legs and upperbody are in the clear, of the 360 degree rotation of the crank handle). This also alows you to use your right hand to crank, and rap your fingers and thumb under the underneath side of crank handle. The crank will come out of your hand better and not pull as much on your arm if it should happen to kick back with fingers and thumb rapped under the underneath side rather than over the top and around to the bottom. Just my two cents worth.
