12.4 is the new size marking used since sometime in the 60s. Prior to that, tires of that size were marked 11-28. 12.4 refers to the cross-sectional width of the tire- not the rim size. 12.4 would be mounted on an 11 inch wide rim, and would measure `12.4 inches at its widest part- the bulge of the tire. 28 refers to the rim diameter or 28 inches-measured where the tire bead mounts on the rim.
