The Case-O-Matic was just a torque convert like you find in a construction equipment like a crawler or loader with the addition of a lockup clutch. At the time they were introduced the issue was fuel economy. They were murder at tractor pulls (most were dead weight back then). Great for starting loads, but they didn't really give a fixed reduction in spead like T/A or some of the other shift on the go alternatives. The issue now would be cost to repair.
