In my experience you start out in a reasonable gear (possibly 2d or 3d if you've got a 4 speed), get her up to speed, disengage the clutch and back off on the throttle, make the shift up to 4th and then re-engage the clutch. Take your time, feed in the clutch smoothly and adjust the throttle to more or less match your ground speed. Away you go. Older tractors tended to have 2 or 3 work gears with a noticeable higher "road gear" that was mostly used for moving it from job to job on a smooth road.
