Oil cooler leaking while running would put oil in the water. If it happened to leak setting then could let water in the oil. To test one just block off the ports and add air to the one side in a pan of water if it doesn't show a leak start adding heat to the water container till it reaches operating temperature. It will start to bubble with about the same pressure as the oil pressure in the engine or up to about a 100 PSI. Yes 100 PSI. I had one on a Cat go bad took about 100 PSI to make it leak with some heat. The oil pressure runs at about 80 PSI driving down the road and idles with about 75 PSI.
