I'm not real sure who made it, I would say Manitou is the place to start.
There should be a capacity and dimensions tag somewhere within the view of the operators seat. It will have a model and serial number, possibly an attachment number. There should also be a serial number stamped in the frame, usually an obvious place, look near the right front wheel.
As for the clutch, those are somewhat generic. A good truck/equipment supply should be able to match it up.
The transmission, won't know until you get in there. Could be something simple like a broken fork, or as bad as trashed gears. I would look into that first before buying a clutch.
There are a lot of aftermarket forklift dealers online. Most only advertise the most popular brands, but have connections to a lot of other resources. Start calling, you might get lucky. There are also lift truck salvage yards around the country.
But try to find some numbers first. It will need to be identified before any parts suppliers can help.
