Burrhead makes a good point. Stop losing your cool. As a holder of boiler license I know low water and operator error are the cause of most boiler failures. That being said, the possibility of poor maintenance habits and the possibility the operator made a mistake which resulted in a tragedy both are points that need to be addressed. Ohio should have a stricter inspection policy. Hopefully, something will come of this.RC
