Since my grandparents, and a lot of other local settlers were from the U.K. maybe that is how the terms became established. We rarely saw stack threshing here. Usually teams of horses and "racks" with field pitchers would haul loads of sheaves in to where the separator was set up in a central location on the farm. They built some impressive straw piles that would serve as bedding and feed for the livestock through a winter or more.
This was my grandparent's outfit in 1910. They formed a threshing syndicate with 6 of them owning an Aultman Taylor thresher and International "Famous" 20 hp portable engine.
