Did he stop, shut it off for some reason and start it from the ground, getting run over in the process? (If safety switches weren't I'm place). I'm having a hard time figuring how you would have fell out a back cab window on any cab unless you were going up a very steep hill or something. We have IH tractors, and the rear windows open wide open, but it'd be a real pain to have to crawl out one let alone fall out. That's the handiest way to get the seat out of the cab by yourself, and it's still not very handy, even with that big back window open.
