For the most part, they are all in the tank. There were a few earlier ones that had the pump mounted on the frame. Dual tank pickups mostly had a frame mounted pump.
Many modern cars also have an access panel under the back seat. That makes replacing a pump a lot less expensive. dangerous, and labor intensive.
Also, FYI, most pumps these days are sold separately from the gauge sender unit. That gauge unit is what makes the pumps cost around $400. A pump alone generally is less than $100. Installation extra.
