To some degree they need to be separate... if you want to be ruthless in terms of cost control you can't be too concerned about the political consequences of your decisions. There's a lot of big ticket items in the system that would be better off situated in regional centers instead of every whistle stop town (within reason)... yet you see decisions being made based on envy and not much else. We're in a situation in this area where they don't have the resources to operate ER's in every one of the old town hospitals but reality is that the resources that are being spent trying to maintain them could be a LOT more effectively laid out in properly operating one central unit with a bit more in terms of advanced care paramedics... but every time they try to do what should be done there's a bloody uprising. So instead they maintain four ER's for an area that could be served by one and the three that are on the fringes are closed half of the time anyway because they don't have staff. That's what happens when management of the system is too close to the politicians...
