When you have a string cut/broken 2 inches from the knot on just the one side it is the tucker/twine fingers not working correctly. They either are nit retracting out of the needle slot or they are missing the twine. Even when they miss the twine most knotters will still tie the knot.
It sounds like yours are lose/worn in the brackets. I would bet that they are missing the twine at speed.
