I did the self cert in July. When I went to the drivers lic. center to do it, the guy looked at my drivers lic. and medical card and said your medical card has one address and you lic. has a different one , Why. Well 911 changed our house number. He said if I would have been stopped it would be a 500 dollar fine. It cost me 18.50 to correct my address. Then he noticed my medical card did not have the day of the month my card expired, so I had to get a new physical. My family doctor said he is no longer going to do DOT physicals because of the nonsense.
