Sometimes it is just a part failure. I bought a WD a number of years ago that a rod through the block. The owner said it was just sitting there idling when it happened but admitted it having low oil pressure. I tore the engine down to salvage any parts and very surprised at what I found. The piston in the dead cylinder moved up and down freely and was not scored at all and the other half of the connecting rod that was bolted to the crank yet looked just fine and spun around perfectly. I unbolted it and the journal was in perfect shape. What had happened was the connecting rod just snapped in two after 50 years of use. Just proves sometimes crap happens no matter what you do.
