No doubt they weren't torqued correctly... I had a set come loose a week ago and destroy the wheels. You know what... it happens. It can happen quickly. Mine were fine an hour before. I just caught them starting to wobble as I came up to a stop sign so I was fortunate enough to catch it before it got the pilots worn out... so I got them tight and got home before changing the wheels. Now I have two new wheels and the torque wrench I should have had before...
