I am just about sure that is the same as mine. The differences in those old ones were in the length of the center piece. One model was for heavier duty work. The one on my M is actually a hybrid I made of the two models. Shafts were the same on the two models.
The company I told you about (Midwest directional) just knew what to send me when I described it. It worked perfectly. They are kind of a "one size fits all" kit. There are seals in there you won't use.
If you order through them the item number is SK122. The price seemed a tad high ($26) but it is perfect. I was shocked at how much easier mine steered and no leaks.
