Twelve years ago, no symptoms but doc found my psa went from 3.8 year before to 4.013. Second test it didn't go down so went to specialist. He suggested a biopsy - said only 1 in 6 chance of cancer but if it was we'd have missed a chance to treat it early. It was, I had surgery and now 11 years later (doc said if I made it 10 years there was less than a 1% chance it would return). My friend, Len, didn't catch it early, his psa was high. Had surgery but to late. Psa kept rising, tried "seeds' - didn't work. Six months after surgery he couldn't walk (tumors pressing on spine) and had chemo. He lasted about a miserable year after that. Died at age 56 (we were about the same age). If you're younger when you get it, it can be really agressive. When older, maybe not so much. In my case, it hadn't gotten out and surgery saved my life. In Len's, maybe where he caught it late, some other treatment may have worked better, I don't know. I just know that you should check you psa every year and if it rises over 4.0 get a biopsy. Some have higher psa but the key is if it rises. Nothing to fool with.
