Was this truck sitting for some time prior to R&R the clutch? maybe there were 'other' problems that contributed to the clutch failure. Such as, but not limited to... faulty MASTER/SLAVE as mike pointed out. Mike pointed to lots of things. The pivot ball the same length? T.O. bearing the same length? They do make several lengths. And I also recall seeing throwout arms having 2 areas where the pivot rides, (for different clutch diameters).
