Looks to me almost exactly like I did at our church last year on the grease trap. The baffles were gone so it was a straight flow through. So I made baffles out of PVC like you have pictured, and extended them out to where you can see them looking down through the opening. I still have a concrete cover however, and I would not feel safe with that little poly cap that you have. I would want a real cover.
