The starter depends upon inertia to engage the drive. The armature must get up to speed quickly so that the drive gear will move on the helix before it starts to spin with the armature. Grease on then helix is sometimes enough to keep the drive gear from moving fast enough.
Other possible causes are a broken drive cushion spring or spring a retaining bolt that has backed out.
