You said you have the bearing cap bolts out, but did you take the bearing caps off? They could be rusted to the block. Does the crank turn? If not, the bearing(s) could be seized to the crank. Or they could have spun. Not much else that could hold the crank in. You definitely should not need a hammer to take it out, especially since you said everything else is off.
