5th gear is direct drive where the input transmission shaft is direct coupled to the rear shaft that drives the differential. The bottom shaft in the transmission has all the other gears that drive the rear shaft. So...it's likely that the constant mesh input gears that drive the lower shaft are stripped. If it is the input gear at the top, it's not a huge job to change that. To find out for sure what is wrong, you will have to pull the transmission cover.
