David, If the shaft is worn, build it up as Dave suggested. If the sleeve is loose in the bolster, you can add shim stock between the sleeve and bolster casting to reduce the free play. The less slop you leave, the better you'll like it. This happened on the bolster of my '51 H and I found that a previous owner had taken a chisel and beat the tar out of the bore in the bolster in an attempt to make the sleeve fit. It was extremely unsatisfactory as the sleeve didn't have firm support and wore away quickly.
