One other thought=-- Have you had the valve cover off to look at the upper head area? Could be a valve out of adjustment or a pushrod jumped out of it's socket. If one of the valves isn't opening or not opening all the way, you'd be getting noise from that particular cylinder. Not likely, but easy to check.
