Do you get smoke out the exhaust while cranking and not starting? If you have no smoke, you're not getting fuel.

If the injection pump head is worn too much and the injector opening pressure was restored to new specs, you may have a situation where the injection pump is not able to develope enough pressure to open the nozzles at cranking speed, but yet will run pretty well once it is started.
