The pipe that is on it now is apparently burned and eroded away. It will be virtually impossible to find a sleeve or shim that will fill the gap and allow the muffler to fit as it should. I suppose you might be able to wrap that pipe with some flashing or maybe even aluminum foil but that repair will not be satisfactory and will have a very short life span. The only real cure is to replace that pipe. The other responders have described what you need. The only other thing I will say is that when you get ready to do that job, be prepared to spend a minimum of 1/2 day doing it, and you will need a 2" pipe tap to recut the pipe threads in the manifold.
