Time for a Clutch?!?!

I was using my H quite abit today to pull logs, and I think to day was the the day that the clutch said game over. I have the it adjusted all the way out to and I cant get the clutch to stop moving, thus alot of gear grinding. The clutch does not slip at all, but it wont release. So I was thinking that I need a clutch and thow out bearing. but will I need a pressure plate? and how hard is this to do on the H? the Only clutch experence I have is with Allis WC.
Is it harder than that? any triks or hints? Thanks for the Help Andrew
