More than likely, your 4/5 slider gear is worn out. You're going to need to find a good used one from the salvage yard, or a new one. This is a common problem because of starting in 4th and shifting to fifth on the go and grinding the gears. The gear that wears out is an internal and is actually like a splne. Also, your bearings may be worn and have play, contributing to the problem. To fix it, input shaft and gear (with two bearings) at front of transmission must come out through the bottom of the clutch housing and the top must come off the transmission. If you decide to tackle this job, get a parts catalog so you can see from the diagrams what you are getting into and also a service manual is a good idea. Gaskets, bearings, the gear and seals to fix this right will end up costing several hundred dollars.
