It Happens all the time. When I had mine a part I did something to help cant remember now it was too long ago But I think that pulling the lever all the way back so it locks is the way they mostly come off.Plus wear, So when you use it pull it back as much as you need dont pull it back till it locks. At least I think thats what makes them come off not 100% sure.
