560 wheel hub clamp bolt

recently noticed that I have a bolt broken off on the rear wheel hub clamp and my wheel has moved a little on the shaft. Bolt looks to be 7/8? Are these bolts supposed to be fine or coarse threads? Also are they a hardened bolt to take the 400 ft lbs torque? And last is it supposed to have a lock washer under the head? Thanks any help is appreciated.
