The two ports on the front can be used for implements that need two cylinders. A front cultivator and a rear mounted plow come to mind. The rear port on the left side can be used also, but was put there primarily for a delayed lift on rear mount cultivators. The hydraulics are not live (unless you have a pump in front of the distributer) and are one-way -- pressure up, gravity down. Don't run two of the ports to a single cylinder, it won't work. If you use a 2-way cylinder with it, you run one hose and vent the down port. There was a factory remote valve (Christmas tree) that gave 2-way hydraulics, or you can use an aftermarket spool valve. The gasoline verision has 25 drawbar HP, there are increased power pistons and sleeves available. If by 12 foot transport disk, you mean a tandem disk the answer is no. The H will handle about a 8 foot tandem disk, if you want any depth at all. We ran a 6 foot heavily weighted in 3rd gear. the wider the disk, the less depth you are going to get. Turn your caps off.
