I�d start by checking the bearing (pilot) on the front of the pinion (top) shaft and inside the input shaft of the transmission. It runs in every gear except in 5th. I�m betting you find your problem here. If that bearing is good, I would check countershaft (lower shaft) bearings moving on to the rest in the rear end. I would also stick a magnet into the grease several places and check for pieces or filings that might give clues as to what is causing it. If you find metal, drain and flush the rear end and replace the grease.
