B surging

When running, the amp guage bounces from end to end and once engine is warm it begins to surge under load and eventually will die. I've done a search on polarity and found how, but my terminals have been painted over and i hate to start scraping away willy-nilly without even being sure that would be the cure. Any thoughts would be appreciated.
