You will need more than just the one new gear. You will also need a new reverse idler gear, since first gear meshes with that, and also a new gear first and second gear for the counter shaft. My parts book for the M shows that the low low first gear is part of the counter shaft where in the regular set up, it is a double gear together with the second gear. I guess that was done because the low low gear is too small to fit on the counter shaft, so IH made it together with the shaft. Thus, you need a new counter shaft with the first gear on it, a new second gear, and a new reverse idler in addition to the sliding 1st gear. The change is not going to be easy or cheap.
