The detents on 656/666/686 are in plate casting on top of transmission, the three shaft/rail that have links up to the shift lever have ball poppets & are spring loaded, maybe you might have a broken spring or rust/grit in that rail/shaft. Other wise the shift cross shaft the handle pushes from side to side has a pin in it that could be worn, also check the bore of lever this pin works against. Next thing left is tooth damage if forks and linkage check out.
