Timing gears on Super A

My understanding is that that when the #1 cyclinder is on the compression stroke at TDC the distributor should have the rotor pointing at #1 on the distributor cap, the camshaft gear that has two dots on one tooth should point directly at the crankshaft timing gear that has one dot. Questions 1) Since the camshaft gear appears to have twice as many teeth as the crankshaft gear, it appears that the crank has to rotate twice for the camshaft to rotate once. So will the engine always be at TDC on compression stroke then the dots align? 2) What would cause the camshaft and the crankshaft to be misaligned, especially if the gears are in very good shape? (found them off by one tooth.) 3) I pulled the camshaft gear off of the camshaft and then tapped it back on. It did not seat all the way to where the camshaft came flush with the face of the gear. This indicates that there is end play in the camshaft. How do I get the camshaft gear to mount flush with the end of the camshaft? I guess it is time to go get a service manual!
