4.6 Ford cold start idle problems

My wife"s gradmothers Linclon Town Barge- 80 thousand or so miles. On a cold start, it will die if you do not keep your foot on it, sometimes it is fine, others it will die as soon as it starts. If you keep your foot slightly on the gas and let it run 1 minute or so you can take your foot off and it will be ok.

Car runs great once warmed. The throttle body and MAF sensor was reportedly cleaned with spray in place a week ago and it was fine for a day. Now it has reverted. Any notions?
 
Sometimes the throttle body and MAF won't respond to a cleaning, they just have to be replaced. I would be very suspicious of the IAC valve. Clean it or buy a new one.
 
Check power to the IAC valve, too. Just went through this on a custom "48 ford truck with EFI. Removed the air injection and EGR and didn"t think, and the wires melted on the header. Blew the fuse and no cold idle, and very slow hot idle.
