Rough idle on start up Chevy 1989

1989 Chevy pickup Silverado very slow coming off of high idle and runs just a little rough first couple minutes.
 
One thing to check would be the gasket between the throttle body and the intake manifold.
 
I have had a couple of vehicles that would not always run smoothly at startup. It helped when I would turn the key to on for a few seconds before starting. This lets the fuel pump pressurize the injector system. I think the fuel system leaks back when the vehicle sits for a day or two especially true on older vehicles. If you go to start immediately some cylinders don't get the correct amount of fuel because the system is not operating at full pressure. This did help my vehicles, may not do the same for yours but doesn't cost anything to try.
 
Like Bob said.Also check for worn out throttle shaft.Can be a big vacuum leak when worn.
 
Ditto on the throttle body to manifold gasket. This is a known problem with these engines. When I replaced mine, the original had completely vanished!
 
also check the idle control valve its on the side of the intake between the air filter and the T/B, you usually can just take it out and clean the tip and inside with some carb cleaner
