Which tractors came with a belt drive PTO on the side or a offset shaft in the back for running belt driven shellers and mills?
 
UK built Fordson Majors / Super Majors had the pulley on the side up to the mid sixties, also seen them on some Internationals. Most older tractors had them, and they were probably an option on some later models. I think you can get a pulley kit to fit on a 540 pto.

We had a (John Deere built) rear pto driven belt drive for new generation Deeres back when we (my Dad actually) custom harvested silage.......
 
Farmall had them up 1956 - my Farmall 200 had pulley attachment on top of differential below seat. Had to remove it to put on attachemnt for top link on 2pt to 3pt hitch.
 
I belived that IH and John Deere were the only ones that offered them as standard equipment till the 60's on the side, and you could get a PTO offset pulley as optional equipment on most other makes
