Most likely the answer is no, but you would have to get the two tractors side-by-side and compare them. It's possible that the 4500 and 345d are related even though they were built 30 years apart.
Do you have the tractor or the loader on hand? Make some cardboard templates of the mounting pads on whichever you have, then go and lay them on the other. If they match, they'll probably work together.
