To start you stick a bar in holes in the rim of the flywheel and pull. The trailing side of the hole is relieved so when it starts the bar will release and not kill you
or stop the engine. I saw one of these some years ago and watched them start it and drive around. Many were used in WW I to pull guns around Europe.
 
I saw a movie on Dish a few days ago titled something like Oklahoma Oil. A wildcat lady was competing against the large oil company in the early days of statehood and oil exploration in the state and had the expected problems. The engine on the rig was a Holt and during the movie there were a few minutes dedicated to just what you said....the bar rolling the engine to get it to start....this one didn't want to start for quite awhile and she and her hired hand wore themselves out in the attempt.
 
sometimes they would kick back and send the bar flying--i was told to never stand on the far side of the machine when someone is starting it
