Not really an accident either a result of stupidity or negligence or both,sounds like no safety harness on and no 2nd person there when around or entering the bin.These were OSHA
regulations when I worked at the concrete plant 20 years ago probably stricter now.

Why do we have to keep hearing about this.There is no reason for something like this to happen. My German ex POW teacher and my Dad. Always told me to never put yourself in danger.If a job requires two people. You get two people. They make safety gear for a reason so use it.

I am not perfect. I have been shocked a few times. Working with power systems.But I was always trying to be careful.When you have to clean up a space. Where some expert was killed. You never forget the smell or what you have seen.I don't understand.

He was just a kid, maybe in a hurry or maybe didn't know any better, hadn't been trained or didn't listen to what he was told, all young men do non-thinking things. Most of us get lucky several times and then get old enough to know we were lucky and then we start paying attention and learn better. Going to be hell on his Parents and other loved ones, he's gone and their lives will never be the same.
