Hauling permits in Ohio

This is my first foray into the permit process, as it was something my brother always handled for the company, but as an individual I am not in their system. I had heard about an annual OS permit, so I checked ODOT's website and found something for $100 that seemed to fit my needs. I called ODOT to set it up and the lady told me to have my insurance co. send them an OS-32 form. My agent at Nationwide (commercial) doesn't know what that is, and asked me if they want proof of insurance. I called ODOT back and told her that I can provide proof of insurance, but she repeatedly told me OS-32 must be filed. Can anyone tell me how to get these two entities together so we can move on to the next step? Thanks, Fritz
 
Google is your friend, Fritz. OS 32 is an endorsement to liability insurance for hauling under special permit (oversize). Available online in a fillable format.
 
I'm in Ohio and get a Over size load permit each year,,it is always a pain to make happen,,when you do get your permit be sure you have the "Instruction sheet" with it in the truck,, the patrol will not recognize the permit with out the instruction sheet with it,,I know this sounds wrong,,but it's true,,and also there is no such thing as a "wide Load" in Ohio,, they are "Over size loads" so when you run your banners be sure you have them the right way.. High way permits and lisencing is a complex ridicules nite mare..and getting worse by the day..
 
I know of several haulers that will not go thru Ohio. When I worked for the Deere dealer we never had permits in Indiana. Only time I got stop was pulling a forest city do all that was about 17 feet folded up.
 
Indiana does not require a permit for hauling oversize farm equipment, unless on an interstate highway. Illinois doesn't require one at all, even on interstate.(tollroads do require a permit in Illinois).
 
Every jurisdiction requires a "conditions sheet(s) to be with the permit in order for it to be valid. Some use a standardized form that accompanies all permits ex. Pa and Ohio and other jurisdictions issue a specific condition sheet for each permit ex Mi and N.Y. With states like Ohio and Pa, we would carry the condition sheet in our license folder and use for multiple loads. In states like Mi and NY, we had to get the new condition sheet for each load. For an annual permit the conditions sheet will be valid for the life of the permit.
 
I know things have changed a lot since I was in the business, a permit use to cost 2.00 for us to move our hd21.
